Neurology and Pain Management Center, PLLC will file insurance claims for its patients as a courtesy. Please bring your insurance card and picture ID with you at the time of your appointment.
In addition, we require that you update your Patient Registration Form that we have on file every 6 months as well as any change in benefit coverage, so that your record is accurate.
Patients are financially responsible for all charges, including deductibles or co-payments. You must make payment in full at the time of service if you have no insurance coverage, unless prior arrangements have been made. For your greatest convenience, we accept cash, personal checks, MasterCard, and Visa.
